Pre-Travel Questionnaire Summary
You are a 23 year old male with nil allergies and taking ventolin inhaler when required for your asthma. You stated that you had vaccinations as a child. You live in Bahrain.
You are traveling to Goa, India on the 12th January 2008 for 5 days staying in a backpackers/1-3* hotel. Nil travel within 6 days prior to the 12th January.
Vaccinations
Travel Health Nurse recommends the following vaccinations are up to date for your travels:
Tetanus/Diphtheria

Poliomyelitis

Hepatitis A

Malaria Risk
There has been recorded cases of falciparum malaria in Goa. Malaria prophylaxis is advised as well as bite avoidance methods discussed in the attached leaflet. The anti-malarial medication that can be taken for this type of malaria are
